Orders for team merchandise are due December 1, 2008. Turn in your order forms and money to Terri Burch in room 181.
Below is a list of items available for purchase. Please make sure to fill out your order form clearly so we can alleviate any mistakes. If you have any questions, don’t hesitate to email me, Terri Burch at tjburc01@louisville.edu.
Anyway, in order to be "green", I am
not sending out catalogs. You may access the catalogs on-line and then get back
to me for a quote. If you need to see what you've ordered in the past, let me

Descriptions:
Style # 8922 - Adult 3-in-1 Systems Jacket (RED) - A heavy winter coat that comes with a removable lining that can be worn on its own. Fleece lining on inside of jacket. These jackets are very nice, very thick, and very warm. Sizes S - 3XL. $100.00/each
Style # 8480 - Quarter Zip Fleece Pull Over (BLACK) - fleece pullover with the team logo embroidered on the chest. $32.00/each
Style #8591 - Polo Ladies - (RED) - short sleeved polo shirt with team logo embroidered on the chest $25.00/each
Baseball Cap (BLACK) - with logo embroidered on the front $12.00/each
Style # 4997 - Jerzee Hoodie (RED) - with logo screened printed on front. $30.00 each
Style # BG97 - Red and black Duffel Bag (RED/BLACK) - The logo will be embroidered on this bag along with your initials. Great size for helmet, boots, etc. Include your initials on the order form. $25.00/each
Style # JP74 - Lined Wind Pants (BLACK) - wind-pants with logo embroidered on thigh Great to wear over your breeches to stay warm at the shows. $30.00/each
Style Glidan - Long-sleeved Tee (BLACK) - long-sleeved tee-shirts with logo screen printed on front and team members of the back w/black writing. MUST HAVE 12 ORDERS placed or cannot have names on the back. $15.00/each
Style Glidan - Short-Sleeved Tee-Shirts (BLACK) - Logo screen printed in white on the front. $12.00/each
